I  hereby apply for admission into the IDOFED Polytechnic Centre for Vocational, Skills Acquisition & Entrepreneurship Education (CVSAE).

I understand that vocational and technical education involves practical activities and that certain training activities may carry inherent occupational risks when safety instructions are not properly followed.

I undertake to:

- Abide by all safety regulations and instructions issued by the Centre and its instructors.

- Properly use all tools, equipment and machinery provided for my training.

- Wear prescribed Personal Protective Equipment (PPE) during practical sessions.

- Immediately report any accident, injury, equipment malfunction or unsafe condition to an instructor or authorised staff member.

- Not operate machinery or equipment without proper authorisation or instruction.

- Accept responsibility for damage caused deliberately or through negligent misuse of Centre property, subject to the applicable rules and procedures of the Centre.

I further acknowledge that the Centre will take reasonable measures to provide a safe learning environment and appropriate safety instructions, but I understand that I also have a personal responsibility to comply with all safety requirements.

I agree to indemnify and hold harmless IDOFED Polytechnic, its Centre for Vocational, Skills Acquisition & Entrepreneurship Education (CVSAE), management, instructors and authorised staff, to the extent permitted by applicable law, from claims arising from my own negligence, wilful misconduct or violation of established safety rules.

This undertaking does not waive any rights or responsibilities that cannot lawfully be excluded.

I hereby declare that the information provided in this application is true, complete and accurate to the best of my knowledge.

I understand that providing false or misleading information may result in the withdrawal of my admission or other appropriate disciplinary action.

I agree that, if admitted, I will comply with the rules, regulations, academic requirements, safety procedures and code of conduct of IDOFED Polytechnic CVSAE.

I understand that submission of this application does not automatically guarantee admission. Admission is subject to the Centre's selection and verification process.
